Kate and Family LLC
By participating in Coaching with Kate and Family LLC, I hereby agree to the following:
1. I agree to assume full responsibility for any risks, physical or psychological injuries or damages, known or unknown, which I might incur as a result of participating in any Coaching or implementing any advice offered by Kate and Family LLC, including traveling to and from an event related to Coaching. For example, if I take advice given in the Coaching and have what I see as a negative result, I assume that responsibility.
2. I knowingly, voluntarily and expressly waive any claim that I may have against Kate and Family LLC, its instructors, agents, and owners, for any injury, death or damages that I or my family may sustain as a result of participating in Coaching or implementing any advice received; INCLUDING LOSS THAT MAY BE CAUSED BY THE NEGLIGENCE OF THE RELEASED PARTY.
3. I release and discharge Kate and Family LLC, its instructors, agents, and owners, from any and all liability, claim, demand or action that I or my family may have related to the loss, theft or damage of any of my personal property while participating in the Coaching.
4. I, on behalf of myself, heirs, and legal representatives, agree to indemnify and hold harmless Kate and Family LLC, its instructors, agents, and owners, from any liability associated with participation in the Coaching, including how I may choose to implement suggestions or skills offered to me.
5. Kate and Family LLC makes no warranty, express or implied, including no warranty of fitness for a particular purpose.
6. If, at any time, I display or disclose intent to harm myself or others, Kate and Family LLC is required to report to the appropriate authorities.
I have read the above release and waiver of liability and fully understand its contents. I am 18 years of age or older and voluntarily agree to the terms and conditions stated above.
